Health-related social needs (HRSN) refer to fundamental requirements that influence an individual's health. This program offers benefits to qualifying members to address basic needs such as assistance with rent and utility payments, tenant support, and storage costs.

Who can use this

  • Members of IHN CCO must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth in the last year, or have a qualifying health condition -Household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avoid homelessness -HRSN housing services can be accessed only once per lifetime.

Documents you may need

  • -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
