Who can use this

  • Members of Jackson Care Connect must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth in the last year, or have a qualifying health condition -Household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.

Documents you may need

  • -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
