Health-related social needs (HRSN) refer to fundamental requirements that influence a person's health. These benefits are designed to assist eligible members with essential needs like rent and utility payment support, tenant assistance, and storage costs. It is important to note that members can access HRSN housing services only once throughout their lifetime.

Fees: There are no fees associated with this service.
Who can use this
- Members of Jackson Care Connect must meet the following criteria: -Aged 65 or older, children aged 0-5, pregnant or have given birth in the last year, or have a qualifying health condition -Household income at or below 30% of the average yearly income in their area -Require assistance to maintain housing -Possess a lease or written rental agreement -Lack resources to avert homelessness.
Documents you may need
- -Medicaid ID number -All pages of a rent/lease agreement -Proof of income for all people age 18 and older in the household
