About this resource
The program acts as court-appointed alternative decision makers for adults who are unable to make some or most decisions related to their basic care and safety. It is considered a last resort, implemented only when no less restrictive alternatives exist to mitigate a significant safety risk and no suitable alternative guardian is available.
Intake Procedure: For more details, visit the website, call, or email. Referrals may come from providers, caregivers, hospitals, or family members. Cases are assessed and prioritized based on urgency of need.
Fees: There are no associated fees.
Who can use this
- Individuals who meet one of the following three criteria: -Those facing immediate or ongoing serious risk of harm or death due to their incapacity -Those who are temporarily safe in a medical or psychiatric facility but cannot be discharged without a guardian's oversight -Those living independently who cannot safely maintain their living situation without guardian support.
