The program provides essential medical and rehabilitation equipment, including power wheelchairs, power scooters, manual wheelchairs with custom seating, disability vans or car lifts, walkers, gait trainers, standing frames, pediatric bath equipment, hospital beds, and pressure-relieving mattresses. However, it does not supply prosthetics or medical supplies and does not cover medical bills or insurance costs.

- Individuals in need of crucial medical or rehabilitation equipment who lack the financial resources or insurance to acquire it from other sources.

- Provide accurate personal and medical information for the completion of the Equipment Evaluation and Request Form.
