The program offers care coordination through phone, text, and email to assist families in navigating available resources. Follow-up is performed to ensure families successfully connect with services. It includes information on general child development and parenting topics, along with referrals to community resources such as early intervention agencies, family therapy organizations, literacy support groups, mentoring and tutoring programs, parenting classes, support groups, and recreational activities.
