About this resource

This program assists older adults and individuals with disabilities in accessing services by evaluating their needs and coordinating with service providers. It also provides extra support for those on Medicaid who may otherwise qualify for nursing home placement.

  • Individuals aged 60 and above, adults with disabilities aged 18 and over, and family caregivers residing in Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, or Asotin counties.
