About this resource
This program assists older adults and adults with disabilities in accessing services by evaluating their requirements and coordinating with service providers. It also provides extra support for Medicaid clients who might otherwise need nursing home placement.

- Individuals aged 60 and above, adults with disabilities aged 18 and older, and family caregivers residing in Kittitas, Yakima, Benton, Franklin, Walla Walla, Columbia, Garfield, or Asotin counties.
