About this resource
This class series is aimed at helping individuals develop healthy habits to prevent type 2 diabetes, specifically targeting those 18 and older who are pre-diabetic or at high risk.
Intake Procedure: Participants can register online for an information session, attend in person, or make a phone call to enroll.
Fees: The full cost of the program is usually covered by Traditional Medicare and some Medicare Advantage plans. For those without Medicare coverage, a sliding fee scale is available.
Who can use this
- Participants must be 18 or older with a BMI over 25 (over 23 for Asian individuals) or at high risk for type 2 diabetes based on specific medical criteria; Y membership is not necessary.
