This program assists individuals whose unhealthy habits, stress, medical issues, or emotional challenges disrupt their daily lives and impact their health. A referral from an ICHS primary care provider is required.
Intake Process: A referral from an ICHS primary care provider is necessary.
Fees: Costs vary depending on the services rendered. The program accepts major private insurance plans, WA Apple Health (Medicaid), and Medicare. It also provides a sliding scale fee structure for low-income clients based on family size and income, ensuring that no one is denied service due to financial constraints.
