About this resource
This class series assists participants in developing healthy habits to prevent type 2 diabetes, targeting individuals aged 18 and older who are diagnosed with pre-diabetes or are at elevated risk.
Registration Process: You can sign up online for an information session, attend in person, or make a phone call.
Costs: The full program cost is generally covered by Traditional Medicare and some Medicare Advantage plans. For those without Medicare coverage, a sliding fee scale is available.
Who can use this
- Participants must be 18 or older with a BMI over 25 (or over 23 for Asians), or those at high risk for Type 2 Diabetes based on specific medical criteria; Y membership is not necessary.
