Case Managers evaluate the temporary and permanent needs of older and disabled adults who are receiving Medicaid in-home personal care services to help coordinate these services for optimal independence.

- Individuals aged 60 and above, or those with disabilities, who pass a needs-based and financial eligibility assessment conducted by the DSHS Home & Community Services office in Lewis, Mason, and Thurston Counties.
