About this resource
Delivers extensive primary care, case management, and home care services for individuals aged 55 and older, in addition to a day health center and the supply of durable medical equipment.
Intake Procedure: Prospective clients should contact the intake representative for a phone screening to determine eligibility. A tour of the facility or a home visit can be arranged to further discuss the program. If the individual decides to enroll, the intake representative will help with the necessary paperwork.
Fees: Enrollment requires being on WA Apple Health (Medicaid), and individuals may also have Medicare/Medicaid.
Who can use this
- Individuals must be 55 years or older, reside in most areas of King or Snohomish County, and be certified as functionally eligible for PACE (or COPES) by DSHS, primarily serving those on WA Apple Health (Medicaid) or Medicare/Medicaid.
