Care Hub links people to nearby health and social services, providing tailored assistance through local Care Coordinators.
Intake Procedure: To begin, either call or fill out an online self-referral form. A Community-Based Worker will perform an intake assessment afterward.
Fees: There are no costs involved.
Who can use this
- The program is available to residents of Ferry, Stevens, Pend Oreille, Lincoln, Spokane, and Adams counties, as well as individuals from the Kalispel Tribe of Indians, the Spokane Tribe of Indians, and the Colville Confederated Tribes who require help with navigating health care, social services, or community resources. Referrals can come from hospitals, healthcare providers, community organizations, statewide systems like 211 or 988, or through self-referral.
