This program offers care coordination for individuals eligible for Apple Health who are placed outside their homes through the Department of Children, Youth, and Families.
Intake Procedure: Referrals can be made by calling or emailing caregivers, caseworkers, tribal members, or the youths themselves who require assistance.
Fees: There are no charges for this service.
