The program provides housing navigation and support services aimed at helping individuals and families locate and keep permanent housing. It includes support in developing essential life skills, obtaining home loans or financing, and creating personal support networks to ensure long-term housing stability. Additionally, it connects participants with community resources for employment, education, healthcare, dental care, and transportation.

Who can use this
- To qualify for services, clients must possess Medi-Cal and active Central California Alliance for Health (CCAH) coverage, with eligible groups including individuals and families facing homelessness or at risk of losing housing within 30 days, victims of domestic violence, adults with Serious Mental Illness (SMI) or Substance Use Disorder (SUD), and high utilizers of the healthcare system.
