About this resource
Health Home Care Management supports families in overseeing the health and wellness of children and youth aged 0-21 who are enrolled in Medicaid. The initiative aims to enhance the health of both children and families while decreasing the need for emergency interventions, absenteeism from school, and hospital admissions. This program is intended to guarantee that children receive the necessary care for their well-being, allowing families to feel secure in managing their child's health.

Who can use this
- Children from birth to 21 years must meet the following criteria: 1) Have active Medicaid 2) Possess a diagnosis of serious emotional disturbance, HIV/AIDS, or other mental health conditions.
