About this resource
This service provides community-based support for adults across Erie County who are dealing with serious and persistent mental health challenges, as well as adults on Medicaid with two chronic health conditions. It includes personalized service planning and offers a range of services, such as 24-hour crisis intervention and connections to medical, psychiatric, social, educational, psychological, and vocational resources in the community, along with both individual and systemic advocacy.
Intake Procedure: Individuals can either self-refer or be referred by a treatment provider.
Fees: No cost.

- Adults aged 18 and older with serious and persistent mental illness or those on Medicaid with two chronic health conditions; specific information can be obtained by calling. Service Area: Erie County.
