About this resource
The program conducts a Health-Related Social Needs (HRSN) Screening, which is a standardized, one-on-one assessment aimed at determining if a Medicaid member has any unmet social needs that could influence their health. These screenings are facilitated by a Social Care Navigator, a trained expert who assists Medicaid members in managing their identified social needs. Navigators may offer Eligibility Assessments, Social Care Plans, referrals, care coordination, and follow-up services. Translation services are available over the phone in more than 180 languages.

Fees: No cost.

- Eligibility is open to anyone enrolled in Medicaid, specifically in Cattaraugus, Chautauqua, Erie, and Niagara Counties.
