About this resource
We operate five Family Health Centers that deliver vital primary care services and act as 'medical homes' for residents in some of Long Island’s most neglected areas. By extending quality primary care and an increasing array of specialty services into the community, we are assisting more Long Islanders in accessing the care they require, precisely where they need it. Our aim is to maintain the health of our patients and prevent hospital admissions. Through Harmony Healthcare Long Island (HHLI), we are introducing a new model of health services tailored for the communities that need them the most. We offer affordable, high-quality health services to everyone in the area, irrespective of their insurance coverage or financial situation. For more information, please call us.
