This program offers case management support to children facing significant mental health challenges or chronic health issues to help them maintain their well-being and avoid emergency medical situations.

Who can use this
- Children from birth to 21 years who have: 1) Active Medicaid 2) A diagnosis of serious emotional disturbance, HIV/AIDS, or other mental health conditions.
