This program offers case management support to adults facing chronic health issues, severe mental health disorders, or HIV/AIDS, helping them maintain their health and avoid hospitalizations.
Intake Process: Referrals can be made by healthcare providers, community organizations, individuals, or family members.

- To qualify, individuals must have two or more chronic health conditions, be living with HIV/AIDS, or have a serious mental health disorder.
