About this resource

This program offers case management and service coordination for the Care at Home initiative. A case manager will support and empower families to utilize a comprehensive range of eligible services and resources to ensure the child can remain at home.

Intake Procedure: Clients are encouraged to call the agency for more information.

Who can use this

  • Available to residents of Nassau and Suffolk, Queens, Brooklyn, and Manhattan, aged from birth to 18 years, who have a significant disability or medical condition.
