East End Disability Associates

This program delivers service coordination for individuals with intellectual and/or developmental disabilities, along with their families and caregivers. Coordinators help these individuals enroll in various programs and obtain essential services, serving both as advocates and sources of support. Documentation confirming the developmental disability is necessary.

Intake Procedure: Clients are asked to call the agency for further information.

Fees: Services are funded through Medicaid, which is a requirement.
