About this resource
This initiative is focused on aiding individuals who are recovering from mental health challenges and addiction to enhance their well-being and achieve their objectives. Services are delivered in both home and community settings and encompass habilitation, psychosocial rehabilitation, peer support, pre-vocational employment, educational assistance, as well as family support and training.
Intake Process: A referral is necessary via the Care Coordination program. Individuals lacking a Care Coordinator should reach out to the Access to Care Program at (631)361-9020 ext. 1136.
