About this resource
The program offers support for individuals requiring help with coordination, including goal setting, health promotion, transitional care, referrals to community and social resources, family support, and advocacy.
Intake Process: Please call for a screening.
Cost: No fees involved.
Who can use this
- To qualify, individuals must: 1) Be enrolled in Medicaid, have Medicaid insurance plans, or be eligible for Medicaid 2) Have a diagnosis of two or more chronic physical conditions, HIV/AIDS, or a serious, persistent mental illness 3) Exhibit significant behavioral, medical, or social risk factors.
