About this resource
Case managers collaborate with clients who are HIV-positive to formulate care plans aimed at improving their self-care capabilities. The services provided may encompass medical and pharmacy case management, respite care, in-home support, advocacy for benefits and health insurance, meal provision, and emergency financial aid. Additionally, referrals can be made for various social services, housing assistance, dental care, mental health and addiction treatment, job placement, and financial planning support.

Fees: There are no charges for services.
Who can use this
- Individuals diagnosed with HIV; emergency financial aid is restricted to those earning below 250% of the federal poverty line.

- ID, proof of Oregon residence, diagnosis confirmation, proof of income, proof of health insurance
