About this resource
Case managers collaborate with clients who are HIV-positive to formulate care plans aimed at promoting their self-care abilities. The services offered for care coordination may encompass medical and pharmacy case management, respite care, in-home support, advocacy for benefits and health insurance, meal provision, and emergency financial aid. Additionally, referrals can be made for various social services, housing, dental care, mental health and addiction treatment, employment support, and financial planning assistance.

Fees: No charges apply.
Who can use this
- People living with HIV; emergency financial aid is restricted to those earning below 250% of the federal poverty level.
