About this resource
Case managers collaborate with clients who are HIV-positive to formulate care plans aimed at improving their self-care abilities. The services for care coordination may encompass medical and pharmacy case management, respite care, in-home support, advocacy for benefits and health insurance, provision of meals, and emergency financial aid. Additionally, referrals can be made for social services, housing, dental care, mental health and addiction treatment, employment assistance, and financial planning.

- Eligibility is for individuals living with HIV, with emergency financial aid available for those below 250% of the federal poverty level.
