Madison Wellness Center By Neighborcare Health
Community Resource
About this resource
Delivers essential health services that include care for minor illnesses and injuries, family planning, physical exams, and health education, exclusively for students enrolled at the clinic school.
Intake Process: To access most services, individuals must fill out a registration form. According to state and federal regulations, minors can consent to family planning, mental health, and substance abuse services independently, although parental consent and registration are highly recommended for these services. For other types of services, a parent consent form and registration are required.
Costs: There are no fees for certain services covered by WA Apple Health (Medicaid), and some private insurance plans are accepted.
Who can use this
- Eligibility is limited to students from the clinic's school or its affiliated partner schools, prioritizing uninsured and underinsured adolescents, while also providing confidential care for those with insurance.
