Intake Procedure: Contact to schedule an appointment. Applicants are required to undergo a nutrition assessment to identify any nutritional risks.

Who can use this
- Eligible individuals include pregnant and post-partum women, infants, and children under age 5 with an income at or below 185% of the Federal Poverty Level.
