- Households facing homelessness or at imminent risk of becoming homeless.
Documents you may need
- Date of birth, name, and Social Security Number for all household members, signed Social Security card, third party homeless certification (if applicable), copy of rental agreement and eviction notice (if applicable), proof of income for the last 30 days for all household members age 18 and older
